Thank you for your interest in enrolling in Three Kings Accident and Medical Clinic. Please read the information and fill the online enrollment form to start the enrollment process.

The Three Kings Accident and Medical Clinic is affiliated with the National Hauora Coalition PHO. If you want this clinic to be your main Healthcare provider you can apply for enrollment. You can be only be enroled at one clinic/General Practice. If you sign up with another clinic, it will affect your eligibility for enrolment funding. You can visit another clinic in an emergency but you may pay higher fees. Only enrolled patients may make an appointment to see the doctor. Appointments are not necessary, but it could make the waiting time lesser.

Enrollments need to be updated every three years, so if you not have needed to see the doctor at this time, you need to call the clinic to update the enrollment or fill in the Re-enrollment request. Sometimes you may need to sign an updated enrollment form.

How to Enrol

Please fill the form online or bring in to the clinic the completed enrollment form and proof of eligibility for funding. e.g. Passport, citizenship papers, NZ birth certificate.

We will need eligibility documents for each member of the family.

Once funding is confirmed – adults pay $19 instead of $85 with under 14 free. Please refer to our Fees page. If you are not enrolled we will not receive government funding to provide cheaper healthcare, which means you end up paying a higher consultation fee.

Funding confirmation through national enrollment services takes 5 business days.

We arrange the transfer of your medical notes from your previous doctor.

You don’t need to nominate a doctor when you enroll as you enrol under the clinic name, but you can prefer to see any doctor you want at your convenience and preference.

It is best if you send in the enrollment form long before you plan to visit as there is no cost involved for enrollment so that we will have your records requested from your previous clinic when you come.

Changes Due To Orange Light

We are open to all services 8am to 8pm 7days a week. 

If you have a booked appointment. Please come on time and inform the front reception or check-in Online and wait in the car till the doctor calls you.

If you are walking in – you can inform the receptionist at the front or call the clinic or Join the queue online.

You must wear a surgical(NOT Cloth) mask use the hand sanitiser and scan the QR code before you enter.

Do not walk in if you are unwell. Call us first or see more information to arrange a virtual consult or to book a face to face consult in our Red Zone at specific times (Usually 1-3pm), if you are unwell with fever, cough, runny nose, sore throat, shortness of breath, loss of sense of smell/taste, diarrhoea, headache, muscle aches, nausea/vomiting or travelled overseas within 14 days or had close contact with a person with COVID19 or work in a quarantine/MIQ facility. We do not see above patients inside the clinic as per Ministry of Health recommendations
