Thank you for your interest in enrolling in Three Kings Accident and Medical Clinic. 
Please read the information and fill out the online enrollment form to start the enrollment process:

The Three Kings Accident and Medical Clinic is affiliated with the National Hauora Coalition PHO. If you want this clinic to be your main Healthcare provider you can apply for enrollment. You can only be enrolled at one clinic/General Practice. If you sign up with another clinic, it will affect your eligibility for enrolment funding. You can visit another clinic in an emergency but you may pay higher fees. Only enrolled patients may make an appointment to see the doctor. Appointments are not necessary, but it could make the waiting time lesser.

Enrollments need to be updated every three years, so if you not have needed to see the doctor at this time, you need to call the clinic to update the enrollment or fill in the Re-enrollment request. Sometimes you may need to sign an updated enrollment form.

How to Enrol

Please fill the registration form online or bring to the clinic the completed enrollment form and proof of eligibility for funding. e.g. Passport, citizenship papers, NZ birth certificate.

We will need eligibility documents for each member of the family. 

Once funding is confirmed – adults pay $19.50 instead of $85 with those under 14 free. Please refer to our Fees page. If you are not enrolled we will not receive government funding to provide cheaper healthcare, which means you end up paying a higher consultation fee.

Please note, that funding confirmation through national enrollment services takes 5-10 business days.

We will request for your medical records be transferred to us from your previous gp practice.

You don’t need to nominate a doctor (register with GP) when you enrol as you enrol under the clinic name, but you can prefer to see any doctor you want at your convenience and preference.

It is best if you send in the enrollment form long before you plan to visit as there is no cost involved for enrollment so that we will have your records requested from your previous clinic when you come.

Re Enrol

If you have not visited the doctor for 3 years or if you have been asked to re enroll, then please click below.

IMPORTANT

Walk-in/Urgent care clinic is for urgent matters and injuries only. Please book an appointment with a GP if it’s for any other reason.
GP Service is by appointment, 8.30-5.30 weekdays for non-urgent matters (AFTER HOURS starts from 5.30pm on weekdays)

X-Ray 9-5pm Weekdays and special hours on weekends

Wearing a mask is not compulsory, but recommended to avoid transmission of infections to you or others.

If you have a booked appointment. Please come on time and inform either the front reception or the GP Wing reception

If you are arriving after 7.00 pm and if the clinic is very busy, we may need to ask you to return the next day or go to Ascot Whitecross at Greenlane, unless it is an Injury/Emergency or an Unwell Child and its safe to do so.
