You will be required to re-enroll at the clinic if you haven’t visited the clinic for 3 years. This is a requirement from the Ministry of Health.
How do I renew / re-enrol
- If the enrollment has not expired, but about to expire, then you can come in for a doctors visit for a general check-up and update your details
- If it has just expired, you can send in the below request first or call us. We will respond back to you, if we need you to fill in a full enrollment and whether you need to send your proof of eligibility details again.
